1. Eligibility
You must be:

- A resident of a U.S. state, D.C., or Guam
- Able to provide accurate information about your identity and health
False or misleading information may result in denial or discontinuation of service.
2. Nature of Services
Amply provides:
- Access to licensed healthcare providers
- Telehealth consultations
- Wellness and medical guidance
- Prescription services (if clinically appropriate)
Amply is not a substitute for emergency medical care.
For emergencies, call 911 immediately.
3. Provider–Patient Relationship
Healthcare consultations are performed by independent, licensed medical providers (“Providers”).
Amply facilitates access but does not practice medicine.
Your relationship is directly with your Provider.
4. User Responsibilities
You agree to:
- Provide accurate, complete, and current information
- Use the Services only for personal purposes
- Communicate openly with your Provider
- Use prescribed medications as directed
- Keep login credentials confidential
Misuse of the platform may result in account termination.
5. Payments and Billing
By purchasing services or treatment through Amply, you authorize:
- Charges to your selected payment method
- Recurring billing for any subscription services (if applicable)
All fees are clearly displayed before checkout.
Amply does not guarantee insurance reimbursement.
6. Refunds
Due to the nature of medical services, refunds are limited.
Consultation fees and medication costs are generally non-refundable once a Provider has completed the review or medication has shipped.
7. Prescription Policy
Prescriptions are issued only if medically appropriate and solely at the Provider’s discretion.
There is no guarantee that a consultation will result in a prescription.
8. Privacy
Your use of Amply is governed by our:

- Cookie Policy

9. Technology Requirements
Telehealth visits require:
- Reliable internet access
- A compatible device
- Up-to-date browser or operating system
Connection issues may impact appointment quality.
10. Termination
Amply may suspend or terminate your access for:
- Violating these Terms
- Providing false information
- Misusing prescriptions
- Behaving abusively toward staff or Providers
You may terminate your account at any time.
11. Disclaimers
We do not guarantee:
- Specific health outcomes
- Treatment success
- Continuous or error-free platform performance
Use is at your own risk where permitted by law.
12. Limitation of Liability
To the fullest extent permitted by law, Amply is not liable for:
- Indirect, incidental, special, or consequential damages
- Provider decisions or medical advice
- Lost profits, data, or opportunities
13. Governing Law
These Terms are governed by the laws of the state in which Amply operates, without regard to conflict of law principles.
14. Changes to Terms
We may update these Terms from time to time. The “Last Updated” date reflects the latest revision. Continued use of the Services constitutes acceptance.
